MAKING AN APPOINTMENT

To make an appointment, you may call our office to set an appointment. If no one is available to answer, please leave a message and we will return your call. Alternatively, you may enter your name and other relevant information on the Contact page of this website and we will follow up with you.

VISITING THE OFFICE FOR THE FIRST TIME

It is recommended that you log onto the Patient Portal prior to visiting the office and complete the new patient forms. This will help to speed up your visit. The Patient Portal will be accessible once you make an appointment and give us your mailing address. Please contact us for assistance if you have not visited the Patient Portal or encounter difficulty accessing the Patient Portal.

WHAT SHOULD YOU BRING?

If you have Medicare or an insurance plan, please bring your insurance card and proof of identity such as a driver’s license. It is very helpful if you write down all medications and supplements (vitamins, health products) that you are taking. If relevant, please contact your former health care provider and request that your records be forwarded to our office.

PUNCTUALITY

Please arrive on time for your appointment, and at least 15 minutes early if you need to prepare forms prior to your appointment. The Practice strives to see patients in a timely manner. Timely arrivals by patients are important for the Practice’s schedule to run on time. If a patient’s arrival is so delayed that it will jeopardize the schedule of the office, the Practice reserves the right of requiring the patient to reschedule the appointment.

CANCELLING APPOINTMENTS

If you are unable to make an appointment, please let us know as soon as possible and no later than 24 hours prior to the time of the appointment. Patients who no show or cancel with less than 24 hours notice are subject to a $50 fee.
